Documents Required for Portugal Passport
As a first-time applicant for a Portugal passport, you will need to submit the following documents:

2. Your original birth certificate.
3. A notarized copy of your birth certificate.

How to apply for Portugal Passport
Portugal offers a variety of passports for its citizens depending on their travel needs. For those who need a passport for travel outside of the Schengen Area, there are two options: the ordinary passport and the passport for public servants. The ordinary passport is valid for 5 years and costs €60, while the passport for public servants is valid for 10 years and costs €120.

If you are a U.S. citizen and you plan to travel to Portugal, you will need to obtain a Portuguese passport. You can apply for a Portuguese passport at the Consulate General of Portugal in the United States.
To apply for a Portuguese passport, you will need to submit the following documents:
